How do I get a medical record from St. Johns County Fire Rescue EMS?
To request a medical record, you must have a signed medical authorization from the patient, the patient’s legal guardian, or a subpoena.
Your request must be in writing and include:
- Approximate time of day
- Patient’s full name
- Address
- Date of the incident
